Before submitting your registration materials, review this checklist!
Rules and Regulations(see menu to the left):
This project conforms to:
__ Size and Structural Specifications (sections 1.0 - 2.0)
__ Biological Safety Standards (section 3.0)
__ Electrical Safety Standards (section 4.0)
__ Chemical Safety Standards (section 5.0)
Registration Materials:
Each project must submit the following: __ Completed and signed Registration From(Team or Individual) __ Completed and signed WRSEF Release Form __ Copy of the 2009 MSSEF Student Checklist (1A, Research Plan, 1B, and Form 1) __ Copies of additional Consent Forms for projects that involve:
__ Non-human vertebrate animals (2, 5A or 5B (1C, 3 if required))
__ Potentially hazardous biological agents (3, 6A or 6B(1C, 2 if required))
__ Controlled Substances (3 (1C, 2 if required))
__ Recombinant DNA (3, 6A or 6B (1C, 2 if required))
__ Human/Animal Tissue (3, 6A and 6B (1C and 2 if required))
__ Human Subjects (4 (1C, 2, 3 if required))
__ Hazardous substances or devices (2 and 3 (1C if required))
__ Scientific Review Committee (SRC) approval for all projects involving research in the above listed area(s)
__ Copies of previous year's Research Plan and Consent Forms, if it is a continued project and Form 7.
