Research Plan Forms 1 a and b
If necessary, Human Consent Form (Form C) and Designated Supervisor Form (Form D)
Due April 16, 2010
Registration Form (Research Plan included)with accompanying approved Forms C (signed by each subject) and Form D, if necessary, are required to enter the Worcester Regional Middle School Science and Engineering Fair.
